Juror qualification questionaire?

Ive been selected for jury duty and they sent me a juror qualification questionaire. Ive heard of many excuses to get out of it. But most of these questions are pretty basic and I cant sound biased or anything by these questions. Is there a way to fill these out somehow without lieing, but still making you look like a bad et to the jury?
